Medical records request tracker template for PI firms

Track each provider request, who owns it and when to follow up. Check for duplicate requests before sending, and keep partial records open until the requested date range and record types have arrived.

Free to copy and adapt. No sign-up.

This template is for administrative tracking only. It is not legal advice.

How to use the template

  1. Use one row per provider request. Split record types into separate rows when they go to different contacts.
  2. Record the authorization, proof of sending and next follow-up date before moving to Sent.
  3. Review overdue requests, partial records, missing authorizations and requests past your firm’s follow-up interval each week.

Download the workbook for all tabs, or use the CSV for a single table. To use Google Sheets, upload the Excel file or import the CSV. Delete the example rows and the final NOTE row in each CSV before adding your own records. Store files in your firm’s approved system and link to them; keep passwords, identity numbers and document contents out of the tracker.

Incomplete check

  • Do the records cover the full date range you asked for?
  • Did every record type come back? Notes, bills and imaging reports often arrive separately.
  • If you asked for a certification or an itemized bill, is it there?
  • Does the page count look right for the number of visits? Note the count in Notes.
  • If anything is missing: set Status to Partial, write exactly what's missing, set a new follow-up date, send to the person in Reviewed by.

Questions before you start?

Can I use this tracker without case-management software?

Yes. Download the Excel workbook or import the CSV into your existing tracker. Assign one owner to each row and agree where supporting documents will be stored.

When should a request be marked complete?

After staff check the received record types and date range against the request. A bill alone does not close a request that also asked for visit notes. Legal assessment stays with the attorney.

Does this set a legal deadline for providers?

No. Ask the attorney responsible for the matter to supply any applicable deadline. The follow-up date is your firm’s administrative review date.

Spend less time chasing client documents.

Runnrr sets up the recurring steps in your existing tools. Start with one workflow and agree the checks with your team.
